Forms & Assessments
BYU requires a series of procedures that help students obtain appropriate clearance to progress in the MPH program. These procedures are met through approval granted by your MPH faculty advisor or BYU administrators given the following forms. Students hold primary responsibility and faculty advisors hold secondary accountability to assure that appropriate department and university forms are complete. Thus, it is recommended that each student and her/his academic advisor regularly review upcoming forms and requirements. They are listed in the general order most students may need them.
Program of Study Forms
Coursework & faculty advisor approval.
- Deadline: 2nd week of winter semester.
Program Study Change
Approval to change coursework.
- Deadline: No specific deadline.
Field Experience Forms
Field Experience Proposal Approval
Used for approval of field experience proposal.
- Deadline: 2 weeks before scheduled meeting and before HLTH 688R (Field Experience) registration.
Final Field Experience Report Approval
Used for approval of final field experience report.

Field Experience (Domestic) Forms
All domestic internships courses require that an internship application is approved by the department and the Academic Internship Office before a student is permitted to add an intership course. In order to register for HLTH 688R (Field Experience) each student is required to fill out an IRAMS internship application to give the university more information about the internship experience and to provide contact information for the university to reach students in case of emergency. Once the IRAMS application is completed and has been approved both by the department internship coordinator and the Academic Internship Office, the flag on the internship course is added, enabling the student to enroll for the course through AIM and MyMap.

Field Experience (International) Forms
*Students are required to visit with the Kennedy Center (422-8241) 3 months prior to leaving for field experience to get all the necessary forms and paperwork completed.
*Note: Once student forms are completed, you will be able to participate in the five required Pre-Departure Seminars conducted at the Kennedy Center. Following Kennedy Center Pre-Departure Seminars, students will be instructed to complete the third form when they arrive in the foreign country.
Field Experience (International) Forms
(All necessary forms are found on the right hand side of the page under "Individual Internships Documents"). Submitted to the Kennedy Center (204 HRCB). 1 July for Fall Semester departure, 1 Nov for Winter Semester departure, 1 March for Spring Term departure, 1 May for Summer Term departure.

Field Experience Log
Must be completed during the supervised field experience.
Institution Review Board Form
This form, also known as human subjects’ approval, must be completed for most circumstance if students are completing mentored research projects. Must be completed and approved before research projects begin.
Graduation Clearance Form
Approval of Completion Form
Approve completion of all MPH program requirements.
- Deadline: Must be completed before clearance for graduation.
Learning Outcomes Assessment Surveys
- Deadline: Must be completed before beginning field experience.
- Deadline: Must be completed within 2 weeks of completing field experience.
Post-Field Work Preceptor Assessment
Emailed by the MPH office to preceptor following completion of the supervised field experience.
Final Self-Assessment/Exit Survey
Emailed by the MPH office prior to graduation.
- Deadline: Must be completed prior to graduation.
